6b: RFP stalled and crashed

I'm mystified by this one. I fuelled up my RFP 742 with 199000lb using RFP's fuel loader, then started the client and hit 'compare' button. Startup, take-off and climb to cruise were OK. I had the fuel panel properly configured for each phase.
About an hour into the cruise the aircraft started to lose altitude. The engines were running as normal. She seemed simply too heavy to stay in the air. A check of the fuel panel revealed why... the fuel on board was now showing 240000lb. Very mysterious. The Flights and Aircraft menu items in FS9 were greyed out as expected and I certainly hadn't interfered with the fuel load since fuelling up as described above.
